Mineola May Days Food Vendor Registration Form

Eligibility and Rules

  • Deadline for booth entry is Monday, April 26, 2010
  • All exhibits must be suitable for family viewing – no offensive items permitted.
  • Exhibitors must furnish own tables, chairs, awnings, lights and equipment.
  • Awnings must be used but visibility cannot be blocked.
  • Spaces will be assigned in order of received registration and payment.
  • After set up, exhibitors must move their vehicles from the exhibit area.
  • No booth breakdown until after 5 p.m. on Saturday, May 8.
  • Mineola Chamber of Commerce reserves the right to cancel the event due to weather conditions.
  • You must have a Temporary Food Permit from the Texas Department of Health. You can mail in a copy of this or you can fax it to the Chamber office at (903) 569-5510. For more information on this, click here.

Lists of all items to be displayed in your booth(s) must be
submitted for approval with your registration form.

HOLD HARMLESS AGREEMENT
Neither the Mineola Chamber of Commerce, Mineola Civic Center or City of Mineola (“Sponsors”) assumes any responsibility or liability or claim for any loss or damages to Exhibitor or Exhibitor’s property arising from Exhibitor’s parti cipation in Mineola May Days. Exhibitor discharges Sponsors from any liability or claim that may arise with respect to any bodily injury, personal injury, illness, death or property damage resulting from any action or inaction by the Sponsors. Exhibitor does hereby release and forever discharge and hold harmless Sponsors from any and all claims related to Exhibitor’s partici pation in Mineola May Days.
