This is the online entry form for the Beautiful Baby & Cutest Kid Contest, part of Mineola May Days.

You can sign up and pay online or you can print the entry form and either mail it or deliver it, and a check for your entry fee, to our office, located at:

Entry is $10 per child with an optional $3 entry for the photo contest.

If you choose to sign up online, please fill out the form completely. After you send the form, a Paypal form will appear that you can use to pay online if you wish. You can sign up online and then bring a check to our office.
